Back on the Bike Thanks to EXOGEN

The below case study is representative of a US FDA approved indication for the use of EXOGEN.

Situation:
Living in the Atlanta-suburbs, Bobby V. is the Director of Finance for a running specialty retailer in the area. Married with two daughters, he enjoys running and cycling in his free time. An avid runner for 45 years, he listens to his body and when he needs a break from the pounding, he gets on his bike to stay fit. In late October of 2016, he was cycling when his front wheel hit an object that wasn’t plainly visible. He hit it so hard that he flew right over the handlebars of his bike.

« I was on the ground, looking up and wondering what had just happened,” said Bobby. “Several bystanders stopped to see if I was alright and one offered to call 911. I thought I was okay and would ride home, but then realized I couldn’t squeeze the back brake handle of my bike. »

His elbow had taken the worst of it. Not wanting to risk riding home injured, he called his wife to pick him up and then decided to put ice on his elbow, thinking he’d feel better in the morning.
Feeling worse the next day, he visited an orthopaedic surgeon in his area who took an X-ray and diagnosed his right arm with a radial head fracture.

In the days following, the pain in his elbow was so bad, it kept him up at night. He visited his doctor a week later and was advised that this was not unusual and his injury was just going to take time to heal. After five more days of continued extreme pain, Bobby decided to get a second opinion.

The second orthopaedic surgeon advised that the diagnosis was correct, but the treatment was not going to foster healing of the fracture, and the key issue was immobilization. Bobby would not heal unless his arm was immobile so his new doctor put him in a full arm cast. He also told Bobby that because of the two week delay in immobilizing the fracture, he may be at risk of a nonunion.

Solution:
Four weeks later, another X-ray showed the joint was still properly immobilized, but did not show any healing. Bobby’s doctor replaced his full arm cast with one that would stretch from his knuckles up to his right elbow so he could better rotate his wrist and not put so much pressure on his elbow joint.

After another four weeks, the X-rays still showed no healing. It was now 10 weeks since his injury and they talked about the reasons why the bone was not healing; including the immobilization delay, the trauma to the bone itself and possible natural lack of blood flow to the area of injury.

Reasons aside, Bobby was running out of options on how he might heal his fracture.

“There were two pieces of bone at the fracture site and neither one showed signs of healing. My doctor began to talk with me about surgery to repair my nonunion,” said Bobby. “But I wanted to avoid surgery and so I asked him what other options might be available and that’s when he asked me if I’d heard of bone stimulators.”

A short while later, Bobby heard from a Bioventus sales representative who met with him to review his X-rays and show him how to use the EXOGEN Ultrasound Bone Stimulator.

“By this time, the doctor had taken me out of a cast and put me into a brace so I would not risk losing full range of motion and I began to use EXOGEN,” said Bobby. “I took this very seriously and had a set time each night for my treatments.”

Six weeks later, he visited his doctor who took X-rays and observed healing on the larger piece of bone that was broken, but not on the smaller piece.

“EXOGEN was working for me and I was showing progress, but my doctor was concerned about this smaller piece of bone,” said Bobby. “He advised me that a CAT scan would provide more detail about this piece of bone that would inform a decision of leaving it in, having surgery to reattach it, or having surgery to remove it.”

Bobby pushed back on the CAT scan and wanted to wait and continue his EXOGEN treatments.

Success:
After another six weeks of treatment. Bobby returned to the office for X-rays.

“My doctor looked at the X-ray and said he saw something,” said Bobby. “When I asked him to be more specific, he showed me the X-rays and as we looked at it from different angles we could see the healing and his exact words to me were, ‘You sir, have dodged the surgery bullet.’”

Learning EXOGEN was working and he could avoid surgery was a great relief to Bobby. He needed this injury to heal as he’d been sidelined too long from that bike accident. By April, after roughly four months of treatment, Bobby was able to do 30 pushups and be much more active than he had been.

“I was released from physical therapy and allowed to do most anything except biking or lifting of heavy weights,” he said. “I continued my EXOGEN treatments until the end of May and learned I was fully healed.”

A key to Bobby’s recovery was his dedication and commitment to following the directions of his doctor, his physical therapist and being compliant in using EXOGEN. He was determined to follow all instructions to the letter so he would have the greatest chance at healing.

EXOGEN Heals Shattered Arm

The below case study is representative of an indicated use for EXOGEN outside the United States and may not be representative of a US FDA approved indication for the use of EXOGEN.

Situation:
James Curtis, his wife and pet cat make their home in Verdun, Quebec, which is a suburb of Montreal. For 32 years, he’s been working as night manager and night auditor for businesses in the area. Then in July 2016, when James was three weeks into a new job as a night manager at a local hotel, he fell into some trouble – literally. After getting off the bus to work James was walking along a sidewalk when he stepped into a rather deep hole that was hard to see in the dark.

“It was unexpected and I went up in the air and landed hard on my arm,” remembers James. “I knew it was broken and I had never experienced so much pain.”

After an ambulance trip to the hospital and an X-ray, James learned that his humerus was broken and that below his elbow, the ulna and radius were shattered.

“On a ratings scale, my satisfaction with EXOGEN is at 10-plus!”

“A surgeon was called in at 4:30 that morning and said that he had never seen anything so bad in his 35 years of practicing medicine,” said James. “He told me they would have to operate within the next eight hours or I would lose mobility in my arm.”

The surgery lasted 4 ½ hours and two days later as he was recovering, a second surgeon on the hospital staff visited him and advised the first surgery hadn’t gone as well as they had hoped. James was advised that he would need additional surgery and it would take place later that day. This second
surgery lasted another 3 ½ hours.

Solution:
After two weeks of recovery in the hospital, James was discharged. Even though he was severely injured, he went back to work.

“I was in tremendous pain and in a cast, but I decided to get back to my job,” said James. “I had just started the new job prior to the accident. They loved what I was doing and told me to come back when I was ready. It was very motivating.”

At this same time, his doctor prescribed the EXOGEN Ultrasound Bone Healing System to help heal the bones in his arm and advised him to begin physical therapy. EXOGEN Helps an Active Hiker Return to the Trails

Situation:
Richard Lee is an environmental services director at a rehabilitation facility in northern Massachusetts. The 62 year-old raised his three adult children in the Northeast while living an active lifestyle vacationing in Cape Cod, riding bikes, and hiking mountains up to 5,000 feet in elevation.

While getting his summer cottage in Cape Cod prepared for seasonal rentals, Richard lost his balance power washing the siding and hit the ground hard.

“I was standing on the third step getting ready to pressure wash the place when I turned to pull the slack on the hose toward me and tried to maintain my balance by twisting my foot as far as I could,” recalls Richard.

As his foot kept turning, he heard a snap and the next thing he knew he was on the ground with immediate pain. Unable to put any weight on it, he went to urgent care for treatment. The doctor onsite took an X-ray and told Richard it was just a sprain, he should stay off it when he could and push through the pain over the next few weeks until it healed.

X-rays taken at the ER showed what Richard had thought all along, his foot was broken. It turned out that he had fractured the fifth metatarsal in his foot, and the misdiagnosis left him at risk of a nonunion. EXOGEN Ultrasound Bone Healing System Helps
Bring Back Life to a Meteorologist’s Knee Situation

Situation:
For almost 12 years, family man Stephen Augustyn has been a civilian meteorologist in the US Air Force at one of the largest bases in the world, located near Omaha, Nebraska. Born and raised in Omaha, Stephen spends most of his time with his wife and two young daughters, reading, watching sports, and, of course, keeping a close eye on the weather!

He was not always limited to being a sports spectator. Stephen grew up playing basketball through grade school and into high school. However, his high school basketball career came to an end at age 17 after several cartilage and bone fragments dislodged from both knees. After eight knee surgeries and numerous X-rays from 1998 through 2013, clinicians eventually confirmed that Stephen had a very rare genetic, metabolic bone disease called hypophosphatasia. To date, he has undergone 15 orthopedic surgeries associated with the disease.

“In one of the surgeries, the doctors had to insert a cadaver graft to fill in an enormous chunk of cartilage and bone that cracked off of the femur in my left knee,” said Stephen.

“EXOGEN stimulated bone healing in someone whose bones were dead. My doctor was stunned at how rapidly I achieved union.”

After the last surgery failed to repair his knee, he consulted another orthopedic surgeon, Dr. David Brown. Dr. Brown decided to save the graft by realigning his knee via a distal femoral osteotomy in May of 2011. The procedure involved cutting 90% of the way through Stephen’s femur on his left knee and inserting a cadaver bone wedge to straighten the knee and eliminate most of its hyperextension.

This prevented Stephen from putting any weight on his leg, left him on crutches for six months, and made everyday living a challenge.

“I was able to work, but it was extremely difficult because everything I do involves sitting at my desk dealing with computers and it was difficult to find places to elevate my leg. I missed a lot of work and wasn’t able to play with my kids due to the pain,” said Stephen

Nearly six months later in follow-up X-rays, Stephen’s doctor confirmed that a significant portion of the osteotomy site was not healing as it should. This was only the second time his surgeon had observed a nonunion associated with a distal femoral osteotomy in 30 years of practice.

A later bone biopsy revealed that his bone metabolism was nonexistent, a condition called adynamic bone disease that had never before been observed in a patient with hypophosphatasia. Zero bone healing, coupled with the fact that he kept having severe pain several months after the operation, prompted Dr. Brown to prescribe EXOGEN.

“I had never heard of EXOGEN until my doctor told me about a form of ultrasonic therapy he wanted me to try,” said Stephen.

An EXOGEN representative met Stephen at his doctor’s office to go over how to use the device and answer his questions.

EXOGEN Helps Heal A Broken Finger

Situation:
Randy Carl Sanders from Morrisville, North Carolina has worked for the city as a street sweeper for more than 15 years. Randy also does various odd jobs on occasion, and one day he and his coworkers were called in to retrieve something out of a storm drain for a citizen.

“We had a hook attached to the storm drain and my coworker and I picked up the grate,” remembers Randy. “He grabbed one side…and I had…the other.”

Accidents happen and in this case a miscommunication with his coworker resulted in the grate, which was six inches thick and weighed 150 pounds, coming down on his left pinky finger.

“Well, when that happened, I really didn’t pay it any attention. I waited about three hours and took my glove off and realized that my finger had turned black and I decided to go get this checked,” said Randy. “I went to urgent care and had X-rays taken.”

“I took the 20-minute treatments at the same time every day. Basically, you can do what you want - walk around, watch TV… sometimes you even forget you’re wearing it.”

Randy’s pursuit of the second opinion led him to a clinician who took new X-rays and compared them to the old X-rays.

“He told me that my finger was broken between the knuckles and not at the tip,” said Randy.

A fracture that fails to improve and requires additional treatment is called a nonunion and Randy’s new doctor suggested two choices for his nonunion fracture – surgery or treatment with the EXOGEN Ultrasound Bone Healing System.

“I had no idea what EXOGEN was, but was willing to try it if it would heal my finger,” said Randy.
